For the Form 14A race box, do not automatically repeat Pakistani: race and nationality are separate fields. Use the applicant's truthful racial or ethnic description as shown in their official records or confirmed by the permitted submitter. If the record does not make that clear, ask the filing route before submission rather than guessing.

Alias is not a substitute for a missing surname. Copy the full name exactly as the passport prints it, and enter an alias only when the passport or another official identity record shows that other name. For a single-name passport, follow the permitted submitter's current field-entry instruction instead of inventing a family name.
